please have a look at
 http://bugzilla.kernel.org/show_bug.cgi?id=4946

aka the bio_clone bug, introduced in mainline kernel 2.6.11-rcX and
fixed in mainline 2.6.12.4 (so it is probably present in debian
2.6.12.2, which you happen to use).

there is also a thread about this in this mailing lists archives,
about August 5.

I'm not saying it is this bug, I just suggest it might be.
